The Italian joined the Black Cats from Arsenal in 2013 and helped them reach the League Cup final in 2014.
Sunderland also announced on Monday that defender Santiago Vergini had left the club to join Argentina giants Boca Juniors in a permanent move for an undisclosed fee.
Vergini made 51 appearances for the Black Cats, though he spent last season on loan at Getafe.
